Below is the form to request your commission on a booking. The booking should be completed first, with the travel supplier. Please take good notes when you are booking with the supplier/vendor. Then enter the details into this form. Do not wait a few days to do this. Do it the SAME day that you book! If you forget to enter in these details, you may not get your commission in a timely manner.
